Preplanning Checklist
The following checklist will help you create your personalized plan.
Ways to Plan Ahead:
General
- Choose a funeral home
- Gather personal information (needed for required legal forms at time of need)
- Make note of any instructions for your services and final disposition
- Jot down obituary thoughts and choose a charity to direct donations to, if desired
Service
- Choose burial or cremation and select desired merchandise
- Choose type of services, locations and a clergy member or officiator to preside over the service
- If desired choose floral arrangements and desired printing, select any musicians whose talents you’d like to employ for music or hymns and select pallbearers
Final Disposition
- If desired choose a cemetery and select a burial plot, mausoleum or niche
- Select a monument or grave marker and inscription.
- Decide who will keep your urn or decide who will scatter your ashes and where

- Your mother’s maiden name
- Your highest level of education completed
- Your usual occupation and type of business or industry
- If a veteran your DD214
- Name, address and phone of person who will be in charge at time of need
